Challenge
Many patients, ages 25-65, with a busy work schedule develop poor eating habits that affect their overall health. The Department and Health Prevention at HFY was looking for a proactive solution that guided and motivated busy patients into living a healthier lifestyle.

Solution
The solution that best suits the patient profile is a blended approach. First, learners will take an e learning course and then they will have two face to face sessions. The first session is for review purposes and creating plans and goals. The second session is a follow up with testing.

Results
The intended outcome is for patients to lower their cholesterol, blood pressure, and sugar levels in a 10 week period by learning how to identify healthy food choices and taking action.
